From Birth to 15 months
- Hepatitis B – First dose at birth, second dose between 1 and 2 months, and third dose between 6 and 15 months.
- Rotavirus –First dose at 2 months and second dose at 4 months.
- Diphtheria, tetanus, and acellular pertussis (DTaP) – First dose at 2 months, second dose at 4 months, third dose at 6 months, and fourth dose at 15 months.
- Hib– First dose at 2 months, second dose at 4 months, and third or fourth dose at 12 to 15 months.
- Pneumococcal conjugate –First dose at 2 months, second dose at 4 months, third dose at 6 months, and fourth dose at 12 to 15 months.
- Inactivated poliovirus –First dose at 2 months, second dose at 4 months, and third dose at 6 to 15 months.
- COVID-19 – A 2- or 3-dose primary series and a booster between 6 and 15 months.
- Measles, mumps, rubella –First dose at 12 to 15 months.
- Chickenpox – First dose at 12 to 15 months.
- Hepatitis A –A 2-dose series from 12 to 15 months.
From 18 months to 18 years
- Hepatitis B –Third dose at 18 months.
- Diphtheria –Fourth dose at 18 months and fifth dose at 4-6 years.
- Inactivated poliovirus –Third dose at 18 months and fourth dose at 4-6 years.
- COVID-19 –A2- or 3-dose series and a booster between 18 months and 17-18 years.
- Influenza (IIVA)–An annual vaccination of 1 or 2 doses between 18 months and 7-10 years, an annual vaccination of 1 to 2 doses of LAIV4 between 2-10 years, or an annual vaccination of 1 dose between 11-18 years.
- Human papillomavirus (HPV) –First dose at 11-12 years.
- Meningococcal –First dose at 11-12 years and second dose at 16 years.
- Meningococcal B –Minimum age of 12 years.
